Ultrix is Digital's first whack at a Unix system, preceding Tru64 et al. V4.0
was based on 4.2BSD according to the Gunkies wiki, and it definitely feels like
it. We're using V4.5, released in 1995, for the VAX, but expect a tour of the
install on MIPS soon. It's noted on the wiki that there are no copies of Ultrix
for MIPS computers (DECsystem and DECstation) available at TUHS. There also
aren't any copies of 4.5, so I had to get a copy from #/osarchive.org, but
fortunately it's identical to the iso used in the wiki install guide. So if you
want to stop here and use that (certainly more polished and well-researched)
guide, feel no shame.
